The Cornerstone Market

Healthy Lives/Healthy Choices, funded by the Health and Wellness Trust Fund’s Heath Disparities Initiative, builds on a history of successful collaboration between The Cornerstone Ministry Inc., Cornerstone Missionary Baptist Church and other local partners to address the disparities in cardiovascular morbidity and mortality among African Americans in Pitt County. The project uses a multi-channeled approach involving policy and environmental changes conducive to a healthier lifestyle in communities, businesses, churches, grocery stores and other community sites. Who is the Target Population? The target population for this project is African American adults living in Pitt County who are potentially at risk for cardiovascular disease. They will be recruited through networking with African American Churches in Pitt County and other community sites such as worksites and grocery stores. What Services Are Provided? On-site Health Screenings (Cholesterol, Blood Pressure, Glucose, BMI) Case Management Education Sessions Smoking Cessation Exercise Classes
